Mr. Smith and the MAS Staff would like to continuosly recognize and reward outstanding students with the MAS "Student of the Month" Program. Potential Students' of the month are based on this criteria:

The MAS staff, fellow students, parents and family may complete this form to nominate students who have displayed exemplary performance in and out of the dojo. Please be as specific as possible as this information will, in a large part, form the basis for Mr. Smith's choice for the award.
